Evernorth Behavioral Health appeals fax number (2026)

Evernorth Behavioral Health, the behavioral network for Cigna Healthcare plans, prints 877.815.4827 as its appeals fax number. Its guide says a person must send each appeal, so automation is refused.

Evernorth Behavioral Health prints 877.815.4827 as its fax number for submitting an appeal, in the contact table of its Administrative Guidelines for providers. Evernorth is the behavioral network for customers with Cigna Healthcare plans. The same guide says appeal requests may not use "automation via electronic faxes or any other submission mediums." Each appeal has to be submitted by a person, and that covers a billing service filing for you.

Which number do I use for an Evernorth Behavioral Health appeal?

The guide's table pairs the fax number with Provider Services at 800.926.2273 and a mailing address for Evernorth Appeals in Chattanooga, Tennessee. Evernorth asks you to call Provider Services first. Have the patient's name, the date of service and the treating provider's name and tax ID ready; many issues resolve on that call. For claims calls, the guide sends providers practicing in California to the California numbers on its Contact Information (Appendix A) page instead.

Fee schedule and multi-patient reimbursement disputes may not need appeals at all. Evernorth sends those through the reconsideration feature on Provider.Evernorth.com, which needs a login.

The contact table warns that call, claim and service channels may differ based on the patient's ID card, so look at the card and the denial letter before you fax. The guide gives plans whose administration Cigna shares with a third party, such as Taft-Hartley trusts, federal employee plans and third-party administrators, their own appeal routes. Clinical denial appeals follow the denial letter, and contract-rate appeals follow the card or that section of the guide.

How long do I have to appeal, and what do I attach?

SituationDeadlineAttach
Payment dispute or denial180 calendar days from the initial decision, or from the last payment adjustmentSupporting documents; for a medical necessity denial, the complete record
Claim denied as late, over 90 days after the date of serviceThe same 180 daysJustification for the delay, such as the EDI transmission report or proof of coordination of benefits
Coverage denial after peer-to-peer review180 calendar days from the claim denial, unless the plan or state differsClinical information, such as the record or a treatment summary

A second-level appeal follows the health plan's or the state's external appeals process and is due within 60 days of receiving the first-level decision letter. In its medical necessity section, Evernorth says most appeals are resolved within 60 calendar days of receipt and that you hear the decision within 75 days, unless state law sets a different time frame. Some out-of-network providers must send an Appointment of Representative form signed by the patient with a standard appeal. Call the number on the ID card to confirm whether yours does.

When Evernorth asks for treatment records, it says to include history, assessments and clinical notes. Psychotherapy notes need care. 45 CFR 164.508(a)(2) requires a patient's authorization to disclose them, with a few listed exceptions, and the definition in 45 CFR 164.501 leaves out summaries of the diagnosis, functional status, treatment plan, symptoms, prognosis and progress to date.

Can I send an appeal from an online fax account?

The guide does not address online fax services that a person operates, so confirm with Provider Services at 800.926.2273 before you rely on one. On page 72 it bars automation, including automation via electronic faxes, and says each individual appeal must be submitted by a person regardless of submission medium, whether by a practice or by a third-party billing service. If Evernorth rejects an appeal as automated, it says it will notify you to resubmit.

Send each appeal by hand, one request at a time, and skip scheduled sends and bulk batches. Upload the appeal as a PDF, enter the number, press send and file the delivery certificate. It records the delivery date, which matters because the deadlines run in calendar days from the denial.

What goes on the cover sheet?

The recipient, Evernorth Appeals. Your practice's name, phone and return fax, the date and the page count. Keep diagnoses, session details and the patient's name off it and on the appeal behind it, in line with the minimum necessary standard in 45 CFR 164.502(b).

For an expedited appeal, call the number on the patient's ID card for filing instructions, and write "expedited" in the reference field of both the fax cover sheet and the appeal. The guide sets the criteria on page 78.

How do I send it from a HIPAA fax account?

Whoever sends the appeal, the online fax account it goes through is a business associate of your practice (45 CFR 160.103), and on Fax Pigeon the BAA is signed at signup on every plan.

A group practice can put the clinicians and the billing person on one number and one inbox, with unlimited people, so a reply that comes back by fax arrives with an AI summary and is assigned to whoever owns the appeal (HIPAA fax for therapists). Plans start at $4.99 a month billed annually for 200 pages sent and received combined (plans and pricing).

What if the fax fails or Evernorth changes the number?

If no delivery certificate arrives, resend once. Then call Provider Services at 800.926.2273, or mail the appeal to Evernorth Appeals at the Chattanooga address in the contact table. Appeals carry a 180-day clock, so do not wait out a failed fax.

Take the number from the current edition of the guide: September 2026, with the code PCOMM-2026-191 at the foot of each page. Older editions are still posted and list more than one appeals line, so a copy saved in a shared drive can mislead. A misdial sends a patient's record to a stranger.

Confirm it on the current form before you send patient information. Fax Pigeon is not affiliated with Evernorth Behavioral Health or Cigna.
