HIPAA fax service with AI summaries of incoming faxes
Yes: Fax Pigeon adds an AI summary to every inbound fax on every plan, from $4.99 a month billed annually, and the summary sits under the same signed BAA as the fax itself.

Yes. Fax Pigeon writes an AI summary of every inbound fax on every plan, from $4.99 a month billed annually for 200 pages, and the summary sits under the same signed Business Associate Agreement (BAA) as the fax. The inbox shows it beside the sender, so your office can tell what arrived before anyone opens the PDF.
What does a summary of an incoming fax give a small office?
Every fax that arrives raises three questions: who sent it, what is it, and what does it need. On a paper machine the answers wait until someone picks up the page. In an inbox they can sit on one row.
| On the inbox row | What it answers |
|---|---|
| The sending number, or the contact name you saved for it | Who sent it |
| A label for the document, such as Prior Auth, Records Request, Referral, Lab Results or Cover Sheet | What it is |
| The person's name, when the page names one | Who it concerns |
| A reference number, when one is printed | What to look up |
| A one-line summary | What it says and what it needs |
That is enough to decide what gets opened first and who gets it. Click a label and the inbox narrows to that kind of document. Click a reference number and it copies. Open the fax and the same summary sits at the top of the detail panel, above the assignment box and the notes. In a shared inbox the front desk can assign a records request to whoever handles records and a prior authorization to whoever calls the payer. The PDF stays one click away.
HIPAA fax for medical billing companies shows the same inbox in a billing office.
Why must the summary sit under the same BAA as the fax?
A summary that names a patient and describes a document is protected health information (PHI) in its own right (45 CFR 160.103). The party that creates it for you is a business associate, because the definition covers anyone who creates, receives, maintains or transmits PHI on a covered entity's behalf. HHS applies the test to vendors directly: a vendor that needs access to PHI to provide its service is a business associate, and the agreement comes first (HHS FAQ on software vendors).
The contract rule is short. A covered entity may let a business associate handle PHI only with satisfactory assurances, documented in a written contract (45 CFR 164.502(e)). HHS says that using a cloud service to process or store electronic PHI (ePHI) without a BAA violates the HIPAA Rules (HHS cloud computing guidance).
Summarizing a fax by pasting it into a general-purpose chatbot sends the record to a second vendor. The BAA question applies to that vendor too, and an answer of no agreement leaves the practice without the written assurance the rule requires.
What should you ask a fax service that offers AI summaries?
- Does the BAA name summarization? Fax Pigeon's BAA lists summarization of inbound fax content among its permitted uses, with document-type classification and metadata extraction, all "for Covered Entity's own use" and under the safeguards the agreement sets for everything else, among them encryption, access controls and an append-only audit log (Sections 4(a)(v) and 3(b)).
- Does it say whether your faxes train AI models? Fax Pigeon's says it does not use identifiable PHI to train, improve or develop any machine-learning or artificial-intelligence model, and does not permit any subcontractor or service provider to use it for that purpose (Section 4(i)).
- Does it bind everyone who touches the summary? The rule requires a business associate to bind its subcontractors to the same restrictions (45 CFR 164.504(e)(2)(ii)(D)). Fax Pigeon's BAA does that and keeps Fax Pigeon responsible for its subcontractors' acts as if they were its own (Section 3(d)).
- Does one agreement cover the fax and the summary? At Fax Pigeon the BAA is signed once, at signup, on every plan.
- Does the summary leave the service? The BAA says email from Fax Pigeon never contains PHI, so a notification carries a page count and a link, not the summary (Section 3(d)).
What does the AI summary cost?
Every Fax Pigeon plan includes it, so it adds nothing to the plan price. Billed annually, Basic is $4.99 a month for 200 pages ($5.99 month to month), Standard is $9.99 for 500 pages, and Max is $159.99 for 10,000. Plans differ only in pages a month and the price of an extra page. The pricing page lists all six. The free first fax sends only, so summaries begin with a paid plan's own number.
Who should switch
An office that opens every fax to find out what it is should switch, along with any practice where the right person has to see the right fax first. Port the number to Fax Pigeon while the old account is open and cancel the old service after it lands, in the order the full porting checklist sets out.

Frequently asked questions
- Is there a HIPAA compliant fax service with AI summaries?
- Yes. Fax Pigeon includes an AI summary of every inbound fax on every plan, from $4.99 a month billed annually for 200 pages. The summary sits under the same signed BAA as the fax, and the agreement lists summarization of inbound faxes among the services it covers.
- Does an AI summary need its own BAA?
- Not with Fax Pigeon, because one BAA covers the fax and its summary. HIPAA treats a summary that names a patient as protected health information, and a vendor that creates it for you needs a written agreement first (45 CFR 160.103 and 164.502(e)). A summary tool outside your fax service's BAA needs its own.
- Does Fax Pigeon use my faxes to train AI?
- No. Section 4(i) of the BAA says Fax Pigeon does not use identifiable protected health information to train, improve or develop any machine-learning or artificial-intelligence model, and does not permit any subcontractor or service provider to use it for that purpose.
- Can I paste a fax into a general AI chatbot to summarize it?
- Only if the chatbot's provider has signed a BAA with you that covers that use. HHS says a covered entity that uses a cloud service to process or store ePHI without a BAA is in violation of the HIPAA Rules. A summary built into your fax service under its BAA avoids the question.
- What does an AI summary of an incoming fax include?
- Each inbound fax gets a label for the kind of document, the person it concerns when the page names one, a reference number when one is printed, and a one-line summary. They sit on the inbox row beside the sending number or its saved contact name, so the front desk sees what arrived before opening the PDF.
