How to fax documents to Social Security: field office or DDS
Fax a local Social Security office at the number in SSA's office locator, found by ZIP code. Fax DDS evidence to the number on its request letter, barcoded letter first, 200 pages at most.

To fax documents to Social Security, use your local office's fax number from SSA's office locator, found by ZIP code. For evidence a DDS or hearing office asked for, fax the number on its request letter with that barcoded letter as page one, no more than 200 pages. A faxed signed application sets only a protective filing date.
How do you fax documents to Social Security?
Faxing documents to Social Security runs on one of two routes: a local field office for papers on a claim, or the number on a DDS or hearing office request letter for the evidence that office asked for.
- Match the papers to their route: a claim the local office handles, or a request letter from a DDS or a hearing office.
- For a local office, enter the ZIP code in SSA's office locator to get the office's fax number and mailing address.
- For a request letter, put that barcoded letter on top as page one and fax to the number printed on it.
- Keep each fax to 200 pages or fewer.
- Check that the sending side shows a successful transmission with the page count.
Each kind of document has its own sender, destination and rule.
| Document | Who sends it | Where it goes | The rule | Official source |
|---|---|---|---|---|
| Documents for a pending claim | The claimant or someone helping with the claim | The local Social Security office, by fax, mail or its drop box | The office's fax number comes from SSA's office locator, by ZIP code | SSA, Submit forms and upload documents |
| A signed application | The claimant | The local Social Security office | A faxed signed application is no longer a valid application; it sets a protective filing date, and SSA then asks the claimant for a signature | SSA notice to advocates, September 19, 2023 |
| A proof document such as a birth record | The record custodian or another SSA office | The office handling the claim | A fax counts as an uncertified photocopy from the custodian and is not acceptable from a claimant or other third party | SSA Program Operations Manual System |
| Medical evidence for a disability claim | Medical providers answering a DDS request | The fax number on the DDS request letter | The barcoded letter as page one; no more than 200 pages; no registration; free | Electronic Records Express FAQ |
| Evidence for a hearing | Attorneys, claimant representatives and medical sources | The hearing office handling the claim, by fax or the secure website | The specific barcode that hearing office supplied for that claimant | Electronic Records Express FAQ |
| Records sent online instead | Anyone who provides health or school records | The Electronic Records Express secure website | No more than 50 megabytes a submission; free | Electronic Records Express |
Which number do you use: the local office or the request letter?
The local office's number from the office locator takes papers on a claim, and the number printed on a request letter takes the evidence that letter asked for.
SSA's page on submitting forms and documents says to find the local office and send documents by fax, by mail or in the office's drop box. SSA's May 1, 2025 blog post says the office's mailing address and fax number come from entering a ZIP code in the online office locator, and that some offices have drop-off boxes.
For evidence, the Electronic Records Express FAQ says to fax the request letter, with the records behind it, to the fax number that appears on the letter. In SSA's staff procedure, POMS DI 22505.030, a DDS asks a medical source to fax the evidence back to the claimant's certified electronic folder, using the fax number for its region, with the bar-coded cover sheet on top of the first page.
What can a fax do at Social Security, and what can it not?
A fax carries papers and evidence to Social Security with two limits: a faxed signed application is no longer a valid application, and a fax of a proof document from a claimant is not acceptable.
SSA's September 19, 2023 notice to advocates says a claimant can still fax an application to establish a protective filing date. SSA then contacts the claimant to obtain a signature.
For documents SSA needs as proof, its policy manual treats a fax like an uncertified photocopy and assumes it is valid when it comes from the record custodian or another SSA office. A fax copy from a claimant or other third party is not acceptable, because no one can tell whether the original was genuine or unaltered. A birth record faxed by the office that keeps it can count as proof, and the same record faxed by the claimant cannot.
How do providers fax medical evidence for a disability claim?
Providers fax medical evidence for a disability claim through Electronic Records Express, with the DDS request letter on top as page one and the fax sent to the number printed on that letter.
Electronic Records Express is a Social Security and state DDS program for sending health and school records on disability claims, online through SSA's secure website or by fax to the state DDS or to Social Security. The fax option works anytime, day or night, no registration is needed to fax, and every Electronic Records Express option is free. SSA says both the online and fax options comply with the goals of HIPAA.
The request letter goes first because its barcode lets SSA match the records to the applicant's disability folder and pay the provider. PC fax software is allowed, as long as the first page of each document is the barcode from the office or DDS handling the claim, which may need to be scanned in. SSA's FAQ says not to fax more than 200 pages.
In SSA's staff procedure, a DDS faxes the request for evidence and an invoice to a source that can send and receive faxes, with an SSA-827 if the source has not already received one. The source need not fax the SSA-827 back. A source that cannot send or receive faxes may be asked for evidence by U.S. mail or another delivery service.
20 CFR 404.1512 sets one follow-up request between 10 and 20 calendar days after the first, with at least 10 calendar days from the follow-up to reply. The Electronic Records Express help desk is 1-866-691-3061, staffed 7am to 5:30pm Eastern, Monday through Friday.
The SSA-827 accompanies every records request, and SSA sends more than 14 million requests a year. A copy, fax or electronic version of the signed form is valid, the form expires 12 months after the date below the signature, and it excludes psychotherapy notes as defined in 45 CFR 164.501. The elements a HIPAA authorization must include and when psychotherapy notes need their own authorization have pages of their own.
How do you know the fax reached the right file?
A fax reached the right file when the sending side shows a successful transmission with the page count and the barcode on page one is the one supplied for that claimant.
SSA's FAQ says the sending fax machine should indicate a successful transmission, including the number of pages sent. Under SSA's staff procedure in POMS DI 81010.090, barcodes index faxed or scanned pages into the correct section of the claimant's certified electronic folder.
In that internal procedure, SSA's own staff report a faxed image still missing from the electronic folder 24 hours after transmission as a failure. A provider may amend a report by sending new information, and the original report stays part of the electronic file. What proves a send on the sender's side is laid out in how to tell if a fax went through.
Who else can send records to Social Security?
Anyone who provides health or school records to Social Security or a DDS can use Electronic Records Express, including school professionals, attorneys and claimant representatives.
SSA's list names medical providers, including hospitals, clinics, doctors and health information managers, beside school professionals and third parties such as attorneys and claimant representatives. A representative's evidence for an electronic disability case, sent by fax or through the secure website, must carry the specific barcode that the DDS or the hearing office handling the claim supplied for that claimant.
The secure website is the route without a fax, and a submission through it cannot exceed 50 megabytes. State Medicaid programs and VA disability exams also ask providers for records, each on its own instructions.
What mistakes delay a fax to Social Security?
The mistakes that delay a fax to Social Security are a missing barcode page, a barcode other than the one supplied for that claimant, more than 200 pages in one fax and the wrong office's number.
SSA warns that without the barcode letter as the first page there may be significant delays in matching the records to the applicant's folder and in paying the provider. With PC fax software, each document needs the claimant's barcode as its own first page. SSA's own staff, faxing paper into a folder, create a separate barcode cover sheet for each document and check that every page is legible and single sided.
A local office's number serves that office, and DDS evidence goes to the number on the request letter.
How do you fax Social Security from a HIPAA fax account?
On Fax Pigeon, a cover sheet is optional, so the barcoded request letter can be page one. A PDF or a phone photo uploads as it is, and how to fax a PDF covers saving a file in that format. Every delivered fax gets delivery proof. The BAA is signed at signup on every plan.

Frequently asked questions
- How do I find the fax number for my local Social Security office?
- The fax number for a local Social Security office comes from SSA's online office locator: enter a ZIP code and it shows the office's mailing address and fax number. SSA says to find the local office and submit documents by fax, by mail or in the office's drop box, and some offices have drop-off boxes. Evidence a DDS asked for goes to the number on its request letter instead.
- Can I fax my Social Security application?
- A faxed Social Security application sets a protective filing date, and SSA no longer accepts a faxed, signed application as a valid application. After a faxed application arrives, SSA contacts the claimant to obtain a signature. SSA announced the change to advocates on September 19, 2023. The local office's fax number comes from SSA's online office locator, found by ZIP code.
- Will Social Security accept a faxed copy of a birth certificate?
- Social Security accepts a faxed proof document such as a birth record when it comes from the record custodian or another SSA office, and it treats that fax like an uncertified photocopy. SSA's policy manual says a fax copy from a claimant or other third party is not acceptable, because it is impossible to tell whether the original document was genuine or unaltered.
- What is Electronic Records Express?
- Electronic Records Express is the Social Security and state Disability Determination Services program that takes health and school records for disability claims, either by fax or through SSA's secure website. Providers need no registration to fax: the barcoded request letter goes first, to the number on that letter, with no more than 200 pages. Every option is free, and the help desk is 1-866-691-3061.
- How do I know Social Security received my fax?
- A fax to Social Security went through when the sending side shows a successful transmission with the number of pages sent, which is what SSA's Electronic Records Express FAQ tells senders to look for. The barcode on page one then indexes the pages into the claimant's electronic folder, and SSA's own staff report an image still missing 24 hours after transmission as a failure.
- Can an attorney or representative fax records to Social Security?
- Yes, attorneys and claimant representatives can fax records to Social Security through Electronic Records Express, or send them through its secure website. Either way, each submission needs the barcode that the DDS or hearing office handling the claim issued for that claimant. SSA lists these third parties beside medical providers and school professionals as people who can use the program.
