Humana medical records fax number (2026)
Humana prints 866-305-6655 as the fax number for medical records it requests for a claim, and 888-815-8912 for disputing a record review. See what to send and how to keep proof.

Humana prints 866-305-6655 as the fax number for the claim-related medical records it requests from providers, on its Medical Records Management policy page. The page, part of Humana's Provider Payment Integrity policies, says "Fax requested records to 866-305-6655." It asks for Humana's request letter with each claim's records. Records Humana requests for a quality program go to the number printed on that request, and disputing a record review finding uses a different number, 888-815-8912.
Which Humana fax number do I use, records or disputes?
| What you are sending | Fax | Where Humana prints it |
|---|---|---|
| Records, itemized bills or invoices Humana requested for a claim | 866-305-6655 | Medical Records Management policy page |
| Records Humana requested for a quality program | The number printed on that request | The request itself |
| A dispute of a medical record review finding | 888-815-8912 | Medical Record Review Dispute Policy page and dispute request form |
Humana lists two other routes for requested records. One is an upload through the Medical Records Management tool in Availity Essentials, under Payer Spaces and then Humana, which Humana says allows faster adjudication of claims. The other is mail. Questions about a request go to 800-438-7885, Monday through Friday, 8 a.m. to 8 p.m. Eastern.
What does Humana want in the packet?
Humana says to send the medical records, itemized bills, invoices and other documentation for the claim, and to attach its request letter for each claim so the records are indexed correctly. It wants everything that substantiates the services and the information needed to adjudicate the claim.
Humana's Medical Record Review Requirements page names the record types it may request. The ones a small practice is most likely to hold:
- Complete office records, with visit documentation, face sheet, patient history, results and correspondence
- Progress notes, treatment notes and evaluations
- Plans of care and treatment plans with updates, signed by a qualified provider
- Psychiatric evaluation notes
- For physical, occupational and speech therapy, notes and logs that show the minutes of each service
- The claim form, either the HCFA 1500 or the UB-04
Without the requested records, Humana says a claim might be denied or adjusted. It calls the denial a technical denial for medical record documentation not received.
What goes on the cover sheet?
The recipient, Humana Medical Records Management. Your practice's name, phone and return fax, the date and the page count. One line saying that this answers a Humana medical record request, with the letter attached, is enough. The patient's name, Humana ID and claim number sit on the request letter behind it.
Humana is a health plan, which makes it a covered entity, and 45 CFR 164.514(d)(3)(iii) lets a covered entity rely, when reasonable, on another covered entity's request as the minimum necessary. Send what the letter lists rather than the whole chart.
How do I send records to Humana from a HIPAA fax account?
The records you send Humana pass through the fax service, a business associate of your practice under 45 CFR 160.103, and Fax Pigeon's BAA with your practice is executed at signup, whatever the plan.
Put each claim's records behind its own Humana request letter and save them as a PDF. Upload it, or send it from your email, enter 866-305-6655 and send. Every delivered fax gets its own delivery certificate, so each claim has its own proof that Humana's number received the pages. Billing offices that answer requests for several practices can work from one number and one inbox with unlimited people (HIPAA fax for medical billing companies).
Plans start at $4.99 a month billed annually for 200 pages sent and received combined (plans and pricing).
Does Humana pay for the copies?
It does, after the records arrive. Humana does not prepay for medical record copies. It reimburses at the contracted rate, and the invoice goes through the same Medical Records Management routes with your HMRM request ID on it. With no rate in your agreement or no contract, Humana pays the CMS rate of 12 cents per page or up to $15 maximum per record. On commercial claims it pays the rate set by state statute.
How long do I have to dispute a record review finding?
Humana offers up to two levels of dispute, and every level goes to 888-815-8912. Unless a contract says otherwise, an in-network provider has 18 months from receiving the original claim determination to file Level One, and an out-of-network provider has 6 months. A Level Two dispute is due within 60 calendar days of the Level One decision letter, and an independent third party may review it. Humana adds that every dispute must meet the time frames in the applicable contract or in federal or state law. If Humana does not receive a written request in time, the original findings are final.
Include the dispute request form that came with the findings letter, and any documents that support your position. The form asks you to mail or fax disputes for multiple claims separately, so send one claim's dispute in each fax.
What if the fax fails or Humana changes the number?
If no delivery certificate arrives, resend once. Then upload through Availity Essentials, where every open request is listed, or call 800-438-7885.
A number saved in an old checklist can send a record to a stranger, so compare the one you dial with the policy page each time. For a dispute, Humana's claim-payment inquiry resolution guide adds that a findings letter naming a different address for submittal takes priority.
