HIPAA compliant fax for optometry offices
An optometry office needs a signed BAA, a fax number that stays on its prescriptions and delivery proof within the FTC's 8 business hours. Fax Pigeon starts at $4.99 a month billed annually.

An optometry office needs a fax service that signs a Business Associate Agreement (BAA), lets it keep the fax number on its contact lens prescriptions and gives delivery proof for each answer to a seller. Fax Pigeon does all three on every plan, from $4.99 a month billed annually for 200 pages. The FTC allows 8 business hours for the answer.
Why does an optometry office still fax?
Contact lens sellers. The FTC's Contact Lens Rule names optometrists among the prescribers it covers (16 CFR 315.2). It lets a seller verify a prescription by phone, fax or email, and the office answers by one of the three too (FTC FAQ). The rule's definition of a contact lens prescription lists the prescriber's fax number, so the number goes home with every patient. The FTC's FAQ also answers the HIPAA question: confirming, correcting or verifying a prescription for a seller the patient designated needs no written authorization, because it is treatment or is required by the rule (45 CFR 164.506(a), 164.512(a)).
Referrals and consults. Dartmouth Hitchcock Medical Center's ophthalmology referral form (Rev. 2026) prints a fax line and a separate medically urgent fax line (referral form). Ohio State's College of Optometry takes a patient referral by fax or by encrypted email (referral page). HIPAA permits a covered provider to disclose protected health information for another provider's treatment activities (45 CFR 164.506(c)(2)), and HHS says that can go by fax, with reasonable safeguards (HHS FAQ).
Diabetic eye exam reports. Premera Blue Cross Blue Shield of Alaska's eye examination report for diabetes asks the eye care professional (boxes for a licensed optometrist, a licensed ophthalmologist or other) to record the dilated exam results and return the form to the primary care provider. Its last line asks whether it went by fax or mail, with a date and initials (Premera form 059030).

How long does an optometry office have to answer a contact lens verification request?
Eight business hours from receipt. If the office has not communicated with the seller by then, the prescription is verified and the seller can ship (16 CFR 315.5(c)(3)). A business hour falls between 9 a.m. and 5 p.m. on a weekday, in the office's time zone, with federal holidays left out. A seller may add the Saturday hours it knows the office keeps regularly (16 CFR 315.2).
The FTC's FAQ works the arithmetic. A request received at 4 p.m. Monday runs out at 4 p.m. Tuesday. One received at 11 a.m. Friday, at an office not regularly open on Saturdays, runs out at 11 a.m. Monday.
To stop an order for an inaccurate, expired or otherwise invalid prescription, the office has to say so before the deadline and give the basis, and the seller then cannot fill it (16 CFR 315.5(e)).
Every answer has to be a direct communication, which the FTC defines as a completed communication by telephone, fax or email. Its FAQ says a fax confirmation sheet showing transmission is enough to confirm a fax is completed, and that sellers do not have to confirm they got your answer.
A seller the patient designated can also ask for a copy of the prescription, and the office has 40 business hours to send it (16 CFR 315.3(a)(3)).
How do you answer a verification request by fax?
- Read the request. It gives the patient, the lens details, the quantity and a contact person's fax and telephone numbers (16 CFR 315.5(b)).
- Check the chart, then fax the answer to that contact person before the window closes: confirm the prescription, send the corrected one, or say it is expired or invalid and why.
- Keep the delivery certificate with the request.
What does an optometry office need in a fax service?
A signed BAA on the plan you buy. A fax service that stores an office's faxes maintains protected health information for it, which makes the service a business associate (45 CFR 160.103). A covered entity may allow that only on satisfactory assurances documented in a written contract or other written agreement (45 CFR 164.308(b) and 164.502(e)). The BAA price comparison shows which services sign the BAA only on a higher plan. Every Fax Pigeon plan includes a signed BAA, executed at signup.
A number that keeps working. A contact lens prescription runs at least one year from its issue date unless the prescriber sets a shorter one on medical grounds (16 CFR 315.6), and it lists your fax number. Port the number in rather than replacing it. A port is free on every paid plan, and your current provider keeps the number working until the transfer lands, which most do in 5 to 15 business days.
An inbox the whole office works. Unlimited people share one number and one inbox at no extra cost. Every inbound fax arrives with an AI summary, and email notifications come per fax or daily. Roles and assignment hand a seller's request to whoever answers verifications and a payer letter to billing.
Proof of delivery. Every delivered fax gets a delivery certificate.
What does HIPAA fax cost for an optometry office?
On Fax Pigeon, pages sent and received count together, and faxes keep arriving past the budget.
Take an office that answers 12 verification requests a week with one-page faxes, sends 12 referrals and exam reports of 3 pages, and receives 8 letters and reports of 3 pages. That is 72 pages a week and about 310 in a month. Standard is the plan for that office: Basic plus 110 extra pages at 10¢ is $15.99 on annual billing, and Standard is $9.99.
The plans below offer a signed BAA, at 200 pages and at 500:
| Plan | Pages a month | Billed annually, per month | Month to month | Signed BAA |
|---|---|---|---|---|
| Fax Pigeon Basic | 200 | $4.99 | $5.99 | At signup |
| SRFax Healthcare Lite | 200 | $10.70 | $12.60 | By request form |
| Fax Pigeon Standard | 500 | $9.99 | $11.99 | At signup |
| SRFax Healthcare Basic | 500 | $13.65 | $16.05 | By request form |
| iFax Plus | 500 | $24.99 | $29.99 | Free on Plus and Pro |
| eFax Business | 500 | $33.33 | $39.99 | Included |
Plus is $16.99 a month billed annually for 1,000 pages. Pro, Premium and Max are on the pricing page. Every plan has every feature, fax from your email included, and works in any web browser, on a computer or a phone.
Who should switch
Switch if the office answers sellers and referrals from a machine in the back, or if its fax service signs the BAA only on a plan above the one you pay for. A per-person price is a reason too. iFax lists its team inbox and assignment on Pro at $39.99 month to month, with a second license at $19.99 a month. eFax Business includes 5 users, then $10 a month for each one after.
Keep the old line active until the number lands; the order of operations for a port has the steps. Head-to-head: iFax, eFax, SRFax.
* Prices last verified October 7, 2026, at each provider's own site. Fax Pigeon is not affiliated with SRFax, iFax or eFax.
Frequently asked questions
- Do optometrists need a HIPAA-compliant fax service?
- Yes, when the practice is a HIPAA covered entity, as an optometrist who bills insurers electronically is (45 CFR 160.103 and 162.1101). A fax service that stores the office's faxes maintains protected health information for it, which makes it a business associate (45 CFR 160.103), and HIPAA calls for a written contract with that vendor (45 CFR 164.502(e)). Fax Pigeon signs one at signup on every plan.
- How long does an optometrist have to answer a contact lens verification request?
- Eight business hours from receipt. If the office has not communicated with the seller by then, the prescription is verified and the seller can ship (16 CFR 315.5(c)(3)). - Can an optometry office answer a contact lens verification request by fax?
- Yes. The FTC says a prescriber must use direct communication to answer that a prescription is accurate or inaccurate, and defines it as a completed communication by telephone, fax or email. Its FAQ says a fax confirmation sheet showing transmission is enough to confirm the fax is completed. On Fax Pigeon, every delivered fax gets a delivery certificate.
- Does a contact lens prescription have to list the office's fax number?
- By the FTC's definition, yes: a contact lens prescription includes the prescriber's name, postal address, telephone number and fax number (16 CFR 315.2). A prescription runs at least one year from its issue date unless the prescriber sets a shorter date on medical grounds (16 CFR 315.6), so the number should keep working. Porting a number in to Fax Pigeon is free on every paid plan.
- Can an optometry office fax referrals and diabetic eye exam reports under HIPAA?
- Yes. HIPAA permits a covered provider to disclose protected health information for another provider's treatment activities (45 CFR 164.506(c)(2)), and HHS says that can go by fax, email or phone with reasonable safeguards, naming a physician faxing a patient's record to a specialist as an example.
